First time patients can contact our Front Desk Coordinator,

What to expect:

Plan to spend up to 90 minutes at your first visit. Your evaluation will take 55 minutes and any miscellaneous time spent with administrative issues can take up to 30 minutes (i.e., completion of forms, scheduling, payment, etc.). All subsequent visits will last for 55 minutes. Plan to arrive 15 minutes early to your first appointment to ensure accurate completion of registration forms. 

Fragrance sensitive envronment:

The rising  incidence of asthma, migraines, and upper respiratory conditions combined with the increased use of powerful long lasting fragranced products has led us to adopt a fragrance sensitive environment . We ask that you refrain from applying spray colognes and scented lotions prior to your physical therapy appointment as the use of such products may trigger allergic reactions and create health problems for other patients and your therapist. Our chemically-sensitive co-workers and clients thank you for not wearing scented products.

Bringing children with you:

Due to the private nature and length of time of the services we provide as well as the intrinsically hazardous environment of some areas of Movement Systems Physical Therapy (MSPT), we do not condone children accompanying patients to treatment.  However, MSPT is committed to promoting a culture which is supportive of the needs patients with family responsibilities. Patients bringing children to physical therapy sessions are requested to adhere to the following policy guidelines:  Policy for children accompanying patients.

Insurance coverage and payable services:

In the state of Washington, patients have direct access to physical therapy services without the need for prescription or referral from a physician or nonphysician practitioner (NPP). However, physical therapy services shall be payable by most insurance companies only with a current prescription, referral, or pre-authorization from a physician or NPP. Be aware that most prescriptions, and all referrals and pre-authorizations have an expiration date and/or visit limit that is separate from your plan's dollar amount or visit limit. Prescriptions are current for 90 days unless otherwise specified.

To determine your coverage and providership stipulations, you must contact your medical insurance company. Coverage and providership stipulations depend upon your medical insurance company and the specific plan you have chosen.  Reading the terms of our contract may feel like you are reading a foreign language text if you are not familiar with the terminology.  MSPT has provided a webpage to assist you in understanding reimbursement terms (please click on MSPT to direct you to this link). 

To assist you in obtaining accurate and pertinent information from your insurance company, we have prepared an insurance benefits worksheet. Please print out and complete this worksheet prior to your first visit.

New Patient Paperwork:

Prior to your first visit, we ask that you download the New Patient Registration Packet and bring the completed forms with you to your first appointment (requires Adobe Reader - see above).

Please be sure to bring the following items with you to your first appointment:

  • The completed forms listed above.
  • Comfortable clothing as noted below:
    • For low back pain or lower extremity problems, please bring shorts and a t-shirt.
    • For females with back, neck, and/or upper extremity problems, please bring shorts and a halter-top, bathing suit top, or one-piece backless bathing suit. Note that a sport back jog bra obscures viewing of the shoulder blades and is not recommended.
  • Your referring provider's updated, written prescription, referral, or pre-authorization.
  • Your current insurance card.
  • A method of payment (i.e., checkbook, credit card, etc.).
